After analyzing and interpreting the data, it is suspected that the transmission pipe of a certain thrust chamber of the unrepaired attitude control power system was damaged, resulting in a decrease in control force and leakage of propellant, abnormal attitude control force, and premature exhaustion of propellant.<\/p>\n<p>At 16:15 on February 1, 2021, the Hyperbola-1 Y2 carrier rocket encountered an abnormality during flight and the launch mission failed. After investigation and analysis, the flight fault was determined to be that a piece of insulation foam that should have fallen off fell on the IV (No. 4) grid rudder. The IV grid rudder was blocked under the action of aerodynamic pressure. The foam was blown off again from the IV grid rudder during the subsequent flight. After investigation and analysis, the cause of the failure of the fairing to not separate was located during the final assembly process. The silicone rubber at the 5# or\/and 6# separation bolt box cover of the longitudinal separation surface of the fairing IV quadrant line adhered to the windshield, causing the fairing to not separate, and thus the flight test did not achieve the expected purpose.<\/p>\n<p>At 15:09 on May 13, 2022, the launch of the Hyperbola-1 Y4 launch vehicle failed. After post-mortem analysis and investigation, the flight fault was located in excess objects, which caused the main valve of the fuel path of the 2000N attitude control engine on the rocket to not be closed tightly, causing a fuel leak. After the fuel was exhausted in advance, the attitude orbit control power system lost the ability to control. The attitude of the third-stage flight section was out of tolerance, triggering active safety control conditions on the rocket. The rocket executed a self-destruct command, and the flight failed.<\/p>\n<p>At 16:30 on December 14, 2022, the Zhuque-2 Y1 carrier rocket ignited and took off at the Jiuquan Satellite Launch Center. The first-stage flight, first- and second-stage separation, second-stage ignition, and fairing separation were all normal. After the second-stage main engine shut down, the downstream aircraft shut down abnormally, and the launch mission failed. After investigation and analysis, the fault was located in the stiffness mismatch between the connection between the oxygen delivery pipe of the traveling engine and the low-pressure housing of the oxygen pump. The low-pressure housing had insufficient strength margin. The low-pressure housing of the oxygen pump ruptured under the action of water hammer during the main shutdown, causing an abnormal shutdown of the traveling engine.<\/p>\n<p>At 12:59 on September 21, 2023, the Ceres-1 Y11 carrier rocket was ignited and lifted off at the Jiuquan Satellite Launch Center. After flying for about 67.5 seconds, the rocket&#8217;s attitude became unstable and the launch mission failed. After analysis and investigation, the flight failure problem was located in the ablation abnormality in the expansion section of the first-stage engine nozzle. During the process of processing the screw holes of the nozzle expansion section assembly, improper operation caused internal defects in the carbon cloth layer, resulting in abnormal ablation of the expansion section, which in turn led to an unstable first-level flight attitude.<\/p>\n<p>At 9:03 on December 27, 2024, the Lijian-1 Y6 carrier rocket was ignited and lifted off at the Jiuquan Satellite Launch Center. The first and second stages flew normally, the separation of the first and second stages, the second and third stages, and the fairing separation were normal. The attitude of the third-stage engine became unstable about 3 seconds after ignition. The autonomous safety control system on the rocket implemented self-destruction, and the launch mission failed. After telemetry data analysis, ground test simulation and troubleshooting tests, the most likely cause of the failure is that during the rework process of the three-stage servo thermal battery, external force caused damage to the internal activation loop wire, and fatigue fracture under the action of mechanical environment such as random vibration during flight, resulting in the failure of the battery pack to output normally, resulting in the failure of the three-stage servo mechanism to act according to the instructions, resulting in attitude instability, self-destruction of the rocket, and mission failure.<\/p>\n<p>At 9:17 on August 15, 2025, the Zhuque-2 E Y3 carrier rocket was ignited and lifted off at the Jiuquan Satellite Launch Center. The voltage of the secondary servo power supply bus dropped abnormally 258 seconds after takeoff, causing the rocket&#8217;s attitude to become unstable. The autonomous safety control system on the rocket self-destructed, and the flight test mission failed.
